Chronic Care Management

Chronic Care Management (CCM) is  practice of analyzing your chronic conditions and developing plans that are geared to improve your overall condition. The breadth of these plans may include traditional or non-traditional forms of care. You play the most important role in developing your care plan. 

The problem for many patients is that they feel shuffled between specialist, not communicated with, and left to figure out complex medical conditions on their own. • Monitor for changes in your conditions

• Two or more chronic conditions (consult with a provider for details)
• Your chronic conditions expected to last at least 12 months
• Your chronic conditions place you at significant risk of death, acute exacerbation/ decompensation, or functional decline
