Transitional Care

 Transitional Care is often needed after discharge from an inpatient setting. This period is often confusing, and can be overwhelming. 

Questions such as, "What doctor do I see?" 

"What medications do I take?" Who is going to help until I can see my regular doctor?" "Who is going to coordinate home health services?" 

We are able to step  in for you to manage your primary care, oversee home health services, and provide guidance to you and your loved ones. We do not replace your regular physician, or interfere with ongoing care that you may be receiving from specialist. We offer Telemedicine visits for transitional care!

•  The health care provider who’s managing your transition back into the community will work to coordinate and manage your care for the first 30 days after you return home. They will coordinate care with you,  your family, and caregiver(s), and with your other health care providers.

                  The health care provider may also:

• Provide information to help you transition back to living at home
• Initiate home health services if needed
• Help coordinate care with other specialists
• Help you with referrals or arrangements for follow-up care or community resources
• Help you with scheduling and managing your medications.

Our services cost the same as your current healthcare services. Even though we offer more, we bill your insurance just as your current medical provider does, and you are responsible for deductibles or co-pays per your insurance plan. The only change is an improvement in quality!
